The world has been using [dental implants](https://www.lodgedental.co.uk/treatment/implants) for longer than you might think. In ancient China, they were made from carved wooden pegs. Even Egyptian kings and Celtic royalty used metal jaw implants, but it was only in recent years that titanium alloys could be secured directly into the bone, one missing tooth at a time. Once implants are fitted, they feel and look identical to the natural tooth. More importantly, they don't require special attention. They're easier to care for than bridges and more comfortable than dentures, but the success of an implant relies on the body's capacity to integrate the titanium into the bone. Osseointegration problems are one of the core reasons for implant failures, but technology has come a long way since it was approved by the FDA in 2015. Today, X-guide technology, ceramic attachments, and Sirona Orthophos scanners have taken implantology to an entirely new level of success. Guided implants have a failure rate that's three times lower than that of free-hand placements, so they're pushing implantology into a brighter future.

# How Dental Implants Work
Dental implants are small, bevelled surfaces that are fused into the jawbone. The tooth crown is attached to the implant, and within a few months, natural bone forms around it. If there are two or more missing teeth, we might use a support bridge construction. Just like traditional bridges, an implant bridge usually relies on a maximum of four supports, which are fitted with a snapping mechanism. This way, we can replace your crown without surgery when needed, thereby reducing dental implant cost. Once implants have been integrated into the bone, they provide excellent stability, allowing you to enjoy even the most resistant foods.

# Are You the Right Patient?
Implants can only succeed in the right conditions. Healthy patients and healthy mouths offer the best odds of success and the lowest dental implant cost. You need enough bone for a successful implant, so if you have low sinuses or significant bone loss, you might need a medical procedure to build up your bone mass before you're a candidate for an implant. Sinus lifts can also help to prevent intrusion. Implants can also fail if the blood vessels in your gum are constricted. Smokers have a failure rate of up to 20%. Tobacco products can also increase the risk of infection and blood clots, so we advise you to avoid tobacco products a few months before and after surgery.

# What to Expect from Surgery
Implant surgery is a relatively easy and pain-free experience that's performed in the clinician's chair. Before your cosmetic practitioner considers it, they will perform a Straumann Orthophos scan to ensure that you're a candidate. A bone graft will be performed, if necessary, usually with synthetic material or bone substitutes. The implant's root will be placed deep into the bone under local anaesthesia. Your clinician may want to wait before placing the crown itself, so you may need to wear a denture for a few weeks. Once osseointegration has taken place, you can finally enjoy your new artificial tooth.

In some cases, an additional surgery will be needed to reopen the gum before the abutment is placed, which adds to your dental implant cost. This additional step will require two weeks to settle before you can finally enjoy your new tooth. You can choose one of two crowns: A removable product includes a plastic gum and a mounted abutment that snaps out of place for daily cleaning. A fixed product is permanently cemented into the abutment. After surgery, you might experience some mild facial swelling and bruising. Now it's up to you to give your implant its best chance of success. The better your oral hygiene, the more likely you'll experience implant success.

# How Guided Implants Work
Dental implants might feel like a routine procedure, but they require an exceptional level of precision and detail. Guided implant placement is helping clinicians to achieve both. Computer-generated guides to improve accuracy while reducing the cost of dental implants. This has a drastic impact on failure rates. When an implant is perfectly planned and oriented according to underlying bone morphology and anatomy, it's more likely to last. The implant is less mobile, so bone can form around it more robustly. This precision also reduces the odds of pain and infection, so fully guided implants have a minimum success rate of 85% within the first five years. We rely on the famous Straumann Implant System and Sirona Orhophos scans. They offer a more detailed idea of bone, teeth, and tissue, so they're perfect for high-risk patients with bone loss. They also allow some patients to enjoy a same-day procedure. Before these innovations crept into the industry, unsuccessful osseointegration drove failure rates up to over 10%.

# The Cost of Dental Implants
The lifetime cost of dental implants is usually lower than that of bridges. If you don't require additional surgical steps, you can expect to pay about £3000 for a single implant. Bridges cost about £400 per tooth—a price that grows with each replacement. Your dental implant cost comes with a crown lifespan of 15 years. The screw itself should never require replacement. In contrast, bridges last between five to 10 years.

# Ceramic Technology
Metal-free restorations have become part of the implantology industry for excellent reasons: Ceramics allow for one-piece implants that are firmly connected to the abutment. Because they're manufactured as a single implant, the entire system is less prone to movement. Titanium is extremely biocompatible, but zirconium dioxide is slowly being absorbed into daily practice for its survival rates. They've become every bit as powerful as titanium alternatives. Their white ceramic root doesn't cause a dark shine at the gum, so they offer a natural aesthetic. In the past, titanium implants could only be used on patients with a gum thickness of 2mm or more. With ceramics, this requirement has been vanquished, so tissue grafts can be minimised. Ceramics also attract less plaque than titanium. They allow excellent blood circulation around the tooth, which encourages tissue health. Introducing Matthew Brennand Roper - Lodge Dental

After qualifying from Bristol in 2007 with distinctions and awards in academic and clinical dentistry, Matthew spent time in practice before heading back into the hospital environment.
 
His early training included rotations in restorative dentistry and oral & maxillofacial surgery at the Bristol Dental Hospital, followed by head and neck surgery, facial deformity and trauma at the Royal London and St Bartholomew’s Hospitals. Dr Brennand Roper completed a clinical masters in prosthodontics at the University of Edinburgh, and two nationally accredited specialist programmes, Prosthodontics and Restorative Dentistry, as a Specialty Registrar whilst at the Edinburgh Dental Institute between 2010 and 2017. He went on to become a Consultant in Glasgow before returning to Bristol in 2022.
 
Dr Brennand Roper is the restorative lead for dental developmental deformities, cleft and hypodontia at the Bristol Dental Hospital and is a Fellow of the Royal College of Surgeons of Edinburgh, the Royal College of Physicians and Surgeons of Glasgow and the International Team for Implantology (ITI). Matthew also has years of experience working as a private referrals specialist.

Step 1 – The dentist will take a full set of photos from different angles, as well as a state of the art 3D scan of your teeth.

Step 2 – These images will then be used to digitally plan & design your new smile and the dentist will work in collaboration with the dental laboratory technician to create a preview.

Step 3 – When you return to the practice, you’ll be able to try the ‘trial smile’ in your mouth. It will simply fit over your existing teeth and you’ll be able to speak, take some photos and see how your new smile is likely to look and feel. # You can get full bridge implants 
A prosthesis (row of false teeth) can be attached to dental implant(s) to replace several or all of your upper or lower teeth. This may be a fixed or removable bridge. The second option means you can take the bridge out the mouth for easy cleaning. It then clips back into place securely for confident eating, speaking or laughing.

# One dental implant can support more than one prosthetic tooth
In fact, a single dental implant can anchor a prosthesis with several false teeth attached. The suitability of this treatment depends on many factors, such as location of the missing tooth and the health/volume of the jaw bone.

# Taking care of your implant(s) with regular dental hygiene appointments can help treatment results last for many years.

# What’s the difference between our whitening treatments?
 
The difference between take-home and in-practice whitening is where treatment takes place and how long it takes to see results. For both, the process begins with digital impressions of your teeth and gums using our state-of-the-art iTero digital scanners. These impressions are used to fabricate custom trays that fit your mouth perfectly – the whitening gel is applied to these trays and placed over the teeth for whitening to begin.
 
If you choose take-home treatment, these trays will be fitted in the practice and you’ll be shown how to apply the whitening gel and place/remove the trays yourself. You will be instructed to treat the teeth in this way for up to 30 minutes twice a day for 2 weeks – so you can fit it around your daily life with ease.
 
In-practice treatment is just that – provided entirely in the practice. This involves a 2-hour appointment, during which your dental professional will apply the whitening solution (don’t worry, you’ll be in a comfortable position and you can still swallow as normal). Mike Gray wins National Clinical Award

James graduated from King’s College London in 2016 and has worked in private practice since that time.

He has chosen to focus his practice on Oral Surgery and Complex Restorative Dentistry, alongside undergoing training in Implant Surgery. He accepts referrals from other dental practitioners for complex/failed extractions and the removal of difficult wisdom teeth.

He has achieved a Postgraduate Diploma in Restorative and Aesthetic Dentistry and has been awarded a Fellowship in the British Academy of Aesthetic Restorative and Implant Dentistry. He also holds a Postgraduate Diploma in Oral Surgery and is close to finishing an MSc in the same field, alongside being a member of the British Association of Oral Surgeons. James’ plan for the future is to focus his practice further on Oral and Implant Surgery so he can provide this to a very high standard in an area of dentistry he is very passionate about. In addition to this, he has been running the Emergency Dental Service Clinics alongside the Local Health Board for the past five years which has exposed him to a large variety of acute dental conditions and their varied presentations, which has been an invaluable experience.

In 2014, Richard won Best Young Dentist UK at the Dentistry Awards and in 2018, he was recognised as an Accredited Member of the British Academy of Cosmetic Dentistry. He was the youngest dentist ever to have achieved this accolade.

Having won multiple awards for his Aesthetic Dentistry, in 2022 Richard was invited to be one of the judges at the Aesthetic Dentistry Awards.

He has a wide range of expertise in cosmetic and aesthetic restorative dentistry including smile design, composite bonding and composite veneers, ceramic veneers & crowns, restoring worn down teeth and removing white spots from teeth. Richard has years of experience in working as part of a multi-specialist team in the treatment of complex cases. It is these skills that landed him on the Tatler recommended dentist list.

In his spare time, Richard enjoys skiing, scuba diving, spending time with his young family and sausage dog!

Richard will be discussing 'The Iconic Solution - Maximising minimal intervention'.

Historically, patients who have been affected by white lesions on their teeth have either had to live with the lesions, or resort to invasive dentistry to mask them. 

Due to modern advancements in dental materials and techniques, white spots can now be removed in a truly minimally invasive way using resin infiltration.

This lecture and hands on will look at how resin infiltration can be used in every day practice to great effect in the treatment of superficial and also deep white spot lesions.

Objectives:

* To recognise the different types of white lesions and their aetiology
* Understand the science behind resin infiltration
* How to treat a simple case
* Complex white lesions-deep infiltration
* Dealing with brown lesions
* Isolation
* Time management and costing cases
